Don't worry to much about the form, that is ( mostly ) just for the first session, and for them to add comments in Japanese. So your next visit will be challenging regardless of who you see. But I would recommend sticking with the mistresses you've seen as they will know more how far to push you.
As for Amsterdam, I'm not sure it is all its made out to be, but perhaps a trip to Berlin if your in the neighborhood. Nina san worked there I think it was last year during the summer.
