My take on this:
-Acceptable advance booking time: no such acceptable timing.
I had been chionging since 1995, the past 20yrs when i feel the need for release i just hoot straight into health centre (then) or call / msg for the ML availability. It can be as impromptu as 5min or as early as 24hr advance booking. Sometimes they might not be available or timing clash, and I will just move on to another place/target.

-Obligation to reconfirm booking: again depending. Me driving, so most time I try to drop a reconfirmation msg to the ML before i start driving to destination. This will give both parties at least a 20-30min advance notice. But there's no obligation to do this. It's just a "buy insurance" practice.
