Deed poll is a document prepared at the Notary Public for the change of name. For example, Nguyen Thi Trinh wants to add Mary to her name officially, she will need to go to Notary Public and the lawyer will prepare a document to change her name officially to Nguyen Thi Trinh Mary. It is only needed if your ba xa has done a change of name before.
