I already gave the main two places that I went to.
Do you speak conversational level Japanese? If so just call the numbers or go to their storefronts. Choose your girl ( if working and available to foreigners - not all are gaijin friendly) then you go to a nearby hotel and she comes to your room.
If you do not speak Japanese then sorry you are out of luck because they will reject you if you cannot communicate in Japanese.
The most popular English supported delivery health in Tokyo is called Asian Mystique but in my opinion they are very overpriced and the girls are substandard in looks. They are popular with the Japan newbies and English only speaking foreigners. Maybe that is why they are so expensive compared to others.
